A meaningful proportion of adults avoid dental care because of fear. And the pattern is almost always the same: something unpleasant years ago, then avoidance, then a problem that grows quietly, then a genuine emergency that confirms the fear. It is a loop, and it is not irrational.

What follows is not reassurance. It is the specific things that change at our BTM Layout and JP Nagar clinics when a patient tells us they are frightened. So you know what you would actually be walking into.

The short answer: Tell us you are anxious before the appointment starts, not during. That single sentence changes how the appointment is run — slower pacing, everything explained before it happens, an agreed stop signal. And shorter first visits designed to build confidence rather than complete treatment.

Tell us before we start, not during

Say it at the desk, or on the phone when you book, or in the WhatsApp message. Before treatment starts, not once you are in the chair and it is already difficult.

It is not an admission and nobody reacts to it as one — a large share of our patients say some version of it. What it does is practical: we allocate more time, plan a shorter first visit. And change how the appointment is run. If you say it mid-procedure, we can still stop and adapt, but you have already had the worst part. If you find saying it out loud difficult, write it in the message when you book. That works just as well.

Everything is explained before it is done

The standard here is that you know what is about to happen before it happens — what the instrument is, what it will feel like, roughly how long it will take. And what the next step is after it.

Fear of the dentist is very often fear of not knowing, of lying back while something happens to you that you cannot see. Narrating it removes most of that. Some patients want the opposite and would rather not hear the detail. And that is fine too — tell us which you are. What we will not do is start something without saying so first. And we will not answer “what are you doing?” with “nearly done”.

You can stop at any point

An agreed hand signal, and it means stop — not “pause when convenient”. Raise your hand and the instrument comes out of your mouth.

This matters more than it sounds. A great deal of dental fear is the loss of control, the sense of being unable to speak or stop what is happening. Knowing you can end it at any second is often enough that you never need to use it. Patients who have a working stop signal report far less distress than patients who do not, even when the treatment itself is identical. We will also break longer treatments into shorter sessions if that is easier for you, even where doing it in one go would be more efficient for us.

Laser treatment means less drilling

Where a laser is appropriate, it can replace the drill for some soft-tissue procedures and certain cavity preparations — which removes both the noise and the vibration, the two things anxious patients most often name.

It is not a universal substitute; plenty of treatment still needs conventional instruments. And we would rather be honest about that than overstate what a laser does. But if the sound of the drill is your specific trigger, say so. And we will tell you whether your treatment is one where the alternative applies. The same goes for the injection — topical numbing gel first, delivered slowly. And we wait until you are genuinely numb rather than starting when we think you should be.

Nobody here will lecture you

Nobody here will comment on how long it has been, or on the state of your teeth. It is a straightforward professional position: lecturing somebody who has just overcome real anxiety to walk through the door guarantees they will not come back, and that helps nobody.

We also do not have to fix everything at once. A first visit can genuinely be just a look and a conversation, with no treatment at all — you leave with a written plan. And you decide what happens next and when. Most people who have avoided dentistry for years find the first appointment considerably less bad than they had built it up to be, and the second one straightforward. If it has been ten years, that is not unusual, and it is not a reason to keep waiting.

Can I just come in for a consultation without any treatment?

Yes, and for anxious patients it is often the right way to start. A consultation is an examination and a conversation — we look, we explain what we find. And you leave with a written plan and costs. Nothing is done that day unless you want it done. Consultations start at ₹300.

Is sedation available for anxious patients?

For most anxious patients, careful pacing, proper local anaesthetic and an agreed stop signal are enough. And that is what we would try first. Where anxiety is severe enough that treatment is genuinely not possible otherwise, we will discuss options with you at the consultation and refer to a specialist sedation setting if that is what is actually needed. We will not talk you into something more invasive than your case requires.

What if I am embarrassed about the state of my teeth?

You will not be the first, or the tenth this month. We see the full range and we treat it as a clinical situation rather than a moral one. There is no commentary, no sighing, and no lecture. If it helps, say at the start that you would rather not hear how bad it is — we will focus on what we are going to do about it instead.

I had a bad experience before. Will you rush me?

No. Appointments are spaced so we are not running behind. Which is part of why the pacing can be slower for patients who need it. If a treatment needs to be split across two shorter visits rather than one long one, we will do that. Tell us what went wrong last time — it is genuinely useful information for planning your appointment.

Does the injection have to hurt?

It should not. Topical numbing gel goes on first and is given time to work, the anaesthetic is delivered slowly — speed is what causes most of the discomfort — and we check you are fully numb before starting anything. If you feel a sensation at any point, say so; needing a top-up is common and is not a problem.

Can I bring someone with me to the appointment?

Yes, and for a first visit after a long gap it often helps. They can sit with you in the surgery. Some patients find it easier to have someone else ask the questions, which is fine too. Just mention it when you book so we know to expect two people. And so we can allow a little more time for the conversation.

Can I listen to music during treatment?

Yes — bring headphones and use them. For a lot of anxious patients the sound of the drill is the specific trigger rather than the sensation. And music or a podcast removes most of it. We will make sure you can still see us and that the agreed stop signal works. So tell us at the start and we will check in visually rather than verbally.

What if I have a strong gag reflex?

Tell us at the start, because it changes how we work — a more upright chair position, smaller instruments, shorter working periods, breathing through the nose. And a topical anaesthetic on the palate for impressions. Digital scanning avoids conventional impression material entirely, which is the worst part for most people with a strong gag reflex. It is common and manageable, and it is not something to apologise for.

Can I get a treatment plan without an examination?

Not a reliable one — a plan without a look is guesswork. What you can get for free is a conversation: describe the problem on the phone or WhatsApp and we can usually tell you roughly what is involved and what range of cost to expect. And we either continue or reschedule — both are normal outcomes and neither is a failure. Knowing this in advance is often enough that it never happens. Because most of the panic in dental treatment comes from feeling unable to stop it.

Will you tell me how bad my teeth are?

Only as much as you want to hear. If you would rather skip the summary and go straight to what we are going to do about it, say so and we will. There is no commentary on how long it has been and no lecture — that approach guarantees people do not come back, which helps nobody. What you will get is a written plan you can read in your own time.
